Transitional care impact of integrated care projects

The PhD project is part of the TRANS SENIOR innovative training network, funded under Horizon 2020. TRANS SENIOR is established to generate evidence regarding care transitions in elderly citizens. It aims to evaluate innovative care models to improve the quality of transitions in order to develop strategies supporting European’s health system challenges. The research studies the transitional care impact of integrated care projects (ICPs) for the elderly population by following the Belgian health care reform plan “Integrated Care for Better Health” (Integreo, https://www.integreo.be/nl), which is initiated to establish a nation-wide system change towards more Integrated care. Therefore, 12 ICP’s have been selected to implement integrated care in order to improve the quadruple aim that focuses on improving quality of care, population health, control health care cost and improve job satisfaction of healthcare providers. External scientific evaluation is performed by six Belgium universities working in a consortium called “Faith” (Federated Consortium for the Appraisal of Integrated Teams in Health in Belgium). Faith will provide support and evaluate the Integreo project and individual ICP’s. The PhD project is nested within the Faith project and will focus on transitional care effects for the elderly in order to benefit the overarching TRANS SENIOR aim.
