Implementation of Machine learning and Personalized Algorithms for patient monitoring after Kidney Transplantation (IMPAKT)

In this multicenter HORIZON EUROPE consortium, we build on home-based applications, non-invasive biomarkers, and innovative image analysis, recently developed by the contributing partners. The project integrates clinical data and advanced molecular profiling of patients newly transplanted at major European kidney transplant centers. It aims to validate the previously developed artificial intelligence (AI) solutions for improved home-based patient monitoring, timely diagnosis of rejection and other injury processes after transplantation, as well as automated disease classification and prognostication, in integrated clinical decision support systems. This will be achieved through a large multicenter clinical trial comparing the advanced monitoring tools with current routine clinical practice. The project is designed to set new standards for patient monitoring after kidney transplantation. Next to direct socioeconomic benefits, ensuing from improved care and outcome of patients with end-stage kidney disease, and from enhancing the European competitiveness in personalized medicine tools for transplantation, it can be anticipated that the AI strategies and monitoring tools are also transferable to other chronic diseases requiring intensive patient monitoring.
