Steps forward in the management of patients with obstructive sleep apnea, toward precision medicine

Obstructive sleep apnea (OSA) is a multifactorial disease with individual expressions, so there is not one therapy or diagnostic tool that fits all subjects. There is a need for a personalized approach in both diagnosis and treatment of OSA. Precision medicine management of OSA is the next step forward in improving care, reducing costs and avoiding unfavorable side effects and comorbidities. The aim of this thesis is to improve the overall effectiveness of OSA treatment, in particular mandibular advancement device treatment. This can be achieved by increasing adherence using an adequate follow-up of fatigue and by improving efficacy using individual treatment optimization by titration and an adequate upfront selection process including both the site, pattern and degree of upper airway collapse and the upper airway collapsibility.
