Chapter 36: Infant regurgitation and pediatric gastroesophageal reflux

Abstract:Guidelines for the diagnosis and management of gastroesophageal reflux (GER) and GER disease (GERD) were published in 2009 and then updated in 2018 by the European and North American Societies for Pediatric Gastroenterology, Hepatology, and Nutrition. In GERD, reflux is associated with troublesome symptoms or complications. The recognition of GER and GERD is relevant to implement the best management. A conservative management is indicated in infants with uncomplicated physiologic GER. Children with GERD may benefit from diagnostic testing and need medical or surgical treatment. During recent years, limited novelties in diagnostic approach and drug treatment have been reported. Pros and cons of multichannel intraluminal impedance have been highlighted. Indications for acid inhibitors have been clarified. Innovations are mainly restricted to the nutritional approach of regurgitation in infants. However, overall “not much has changed” in the diagnosis and management of GER and GERD in infants and children.
