Supporting and strengthening informal care-groups and care-networks. Guidance for the health professional.

In this project, a care model is outlined to support and strengthen the informal care-group in the care process. The project focuses on the functioning of this group, the strengthening of the healthcare networks in the absence of informal care and the intervention by the professional care provider.

The target group are health professionals focusing on the following two groups: care-dependents supported by a informal care-group and vulnerable care-dependents without or with a limited network.
